29 CFR 1926.600(a)(6)(i): For lines rated 50kV or below, a minimum of 10 feet was not maintained between the energized line and the equipment. On or about June 29, 2023, on the northeast side of the jobsite; employees were depositing paving material when the boom of the concrete pump truck that was positioned within 10-foot made contact with energized power line line rated at 50kv or below.

29 CFR 1926.600(a)(6)(iv): A person was not designated to observe clearance of the equipment and give timely warning for all operations where it is difficult for the operator to maintain the desired clearance by visual means: On or about June 29, 2023, on the northeast side of the jobsite; employees were depositing paving material without a designated spotter when the boom of the concrete pump truck that was positioned within 10-foot made contact with energized power line.
