29 CFR 1926.416(a)(1): Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: a) At or near Mallory Ave., Jersey City NJ - Slab Edge Parralel to Mallory Ave: employees were exposed to electrocution hazards while engaged in slab forming, rebar installation and concrete placement approximately 6 feet away from an energized 13KV line, from about 12/2/24 to 3/11/25.

29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall, was not guarded to eliminate the hazard of impalement: a)At or near 1 Mallory Ave., Jersey City NJ - Perimeter of Slab - Ground Level: Employees and other workers were exposed to empalement hazards while engaged in various activities over approximately 3 feet long rebar pieces, including but not limited to installation of forms, rebar and concrete, for a period of 2/14/25 to 3/3/25. b)At or near 1 Mallory Ave., Jersey City NJ - Perimeter of Slab - Ground Level: Employees and other workers were exposed to empalement hazards while engaged in various activities over approximately 3 feet long rebar pieces, including but not limited to removal of concrete blankets and adjustble steel shoring poles, on or about 3/11/25.
