OSH ACT of 1970 Section 5(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees who were exposed to struck by hazards:   (a) On August 8, 2025, at 210 64th Street SE in Minot, North Dakota, while in the process of conducting the hydrostatic testing of a high pressure hose, the employer failed to ensure that the operator and other employees in the immediate vicinity were protected from inadvertent struck-by hazards. The 15,000 PSI rated hose was pressurized to over 21,000 PSI when the high-pressure hose ruptured.  Note: Among other methods, feasible and acceptable abatement methods to correct this hazard include but are not limited to:  Expand the existing Standard Operating Procedures for conducting hydrostatic testing to address equipment that cannot fit into the hydro test bay and provide for and require that signs and barricades, or other protective barriers, are placed in a manner and at a distance sufficient to demarcate a safe zone to protect personnel from unanticipated pressure release or equipment failure. The procedures should include a requirement to conduct a Job Hazard Assessment before performing hydrostatic testing to ensure the Standard Operating Procedures can be followed for the assigned task, with clear language stating the testing cannot be conducted unless all of the Standard Operating Procedures are followed and outlining how RDI management will be informed when testing cannot be conducted per the Standard Operating Procedures.
