Location: Job site; On or about 08/12/2025, employees were allowed to perform roofing work on a residential pool house roof that was approximately 20-feet above the ground below without any form of fall protection, such as guardrails, safety nets, or alternative protective measures.

29 CFR 1926.1053(b)(6): Ladders were used on surfaces which were not stable and level. Location: Job site; On or about 08/12/2025, employees were allowed to utilize a Werner portable extension ladder which was positioned near the edge of a small dirt ditch which had white PVC water piping at the bottom of it and consisted of dirt and other loose aggregate excavated materials.
