29 CFR  1926.102(a)(1): Eye and face protective equipment was not used when machines or operations presented potential eye or face injury:   At the jobsite located at 9528 Antioch Road, Overland Park, Kansas, employees engaged in concrete cutting activities using a handheld concrete cut-off saw, were exposed to struck by hazards in that eye protection was not utilized.

29 CFR  1926.1153(c)(1):For each employee engaged in a task identified on Table 1, the employer did not fully and properly implement the engineering controls, work practices, and respiratory protection specified for the task on Table 1.   At the jobsite located at 9528 Antioch Road, Overland Park, Kansas, employee(s) dry cutting on concrete were exposed to silica hazards. The employer failed to implement engineering and work practice controls as outlined in Table 1 to reduce employee exposure to silica dust.
