Jobsite: On or about August 13, 2025, the employer did not ensure that an employee chose the correct ladder for the intended purpose. The employee used a fly section of a 24-foot extension ladder with no feet as a straight ladder. The employee suffered injuries from a fall when the base of the ladder slipped out.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards. Jobsite: On or about August 13, 2025, the employer did not provide adequate ladder safety training to ensure that an employee recognized the hazards of using the fly section of an extension ladder without slip resistant feet as a straight ladder. The employee fell approximately eight feet when the bottom of the ladder slipped out at the base.

Jobsite: On or about August 13, 2025, the employer did not ensure that an employee selected a ladder with rubber feet when working from it on a slippery surface. The employee used the fly section of a 24-foot extension ladder that did not have rubber feet and fell approximately eight feet when the bottom of the ladder slipped out.
