OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:  a) Tech Dynamics Inc. located at 361 D Street, Perrysburg, Ohio, 43551: On or about August 13, 2025, the hook of the R&M hoist on the 5 ton Bohl overhead crane located in the south shipping/receiving bay area did not maintain a functional safety latch mechanism, effectively closing the hooks' throat.  Employees operating the overhead crane in this condition to transport steel products weighing 500-1,400 lbs were exposed to crushing injuries associated with displaced loads.  Among other methods, feasible and acceptable methods to correct the hazardous condition may include:  1. American Society of Mechanical Engineers (ASME) ASME B30.16-2012 Standard Section 16-1.2.9 Hooks. The employer must ensure that hooks are equipped with latches to bridge the throat opening of the hook, and to retain, under slack conditions, such as items as, but not limited to, slings and chains.   2. American Society of Mechanical Engineers (ASME) ASME B30.10- 2019 Standard, Section 10-1.10.6 Repairs and Modifications: (c) A hook latch that is inoperative shall be repaired, replaced, or removed if not required, (d) if a required latch is inoperable and cannot be immediately repaired or replaced, the hook shall be sufficiently moused to retain loose items as defined in para. 10-1.3(c) until the latch is repaired or replaced.

C, Perrysburg, Ohio, 43551: On or about August 13, 2025, the employer failed to ensure that a hazard assessment identifying and selecting appropriate personal protective equipment was conducted prior to employees performing work activities such as, but not limited to, overhead crane operations and loading/unloading of trucks.  Employees performing these operations without the benefit of an employer assessment, were exposed to head and other bodily injuries from falling load hazards.
