On or about August 13th, 2025, it was observed that employees engaged in residential roofing operations on the job site located at 20073 Water Lily Street, Elkhorn, Nebraska 68022, were exposed to fall hazards in that the employer had an employee working at heights greater than 11 feet 8 inches without any form of fall protection in place.

29 CFR 1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: On or about August 13th, 2025, employees engaged in residential roofing operations on the jobsite located at 20073 Water Lily Street, Elkhorn, Nebraska 68022, were exposed to fall hazards in that the employer failed to train his employee on the hazards related to working at elevation and the use of fall protection equipment prior to exposing the employee to hazardous heights in excess of 6 feet.
