On or about 08/12/2025, Uptime Cooling Systems, LLC failed to ensure employees operating an electric Kilns FL-27E Oven were free from shock hazard. The heating element of the Kilns FL-27E was broken exposing employees to 240 volts while placing a diesel particulate filter in the oven.

On or about 08/12/2025, Uptime Cooling Systems, LLC did not inspect an electric Kilns FL-27E Oven for damaged parts such as broken heating element to ensure safe operation of the oven. The employees were using this oven daily to place DPF filters for thermal cleaning while exposed to shock hazards of 240 volts.

On or about August 13, 2025, Uptime Cooling Systems, LLC employees were observed using an extension cord that was spliced and had damaged insulation to power a DeWalt Angle Grinder. The employer failed to visually inspect the extension cord before each use for external defects such as loose parts, deformed and missing pins, or damage to outer insulation, and for possible internal damage such as pinched or crushed outer jacket.
