29 CFR  1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provided for an alternative fall protection measure:  a) Job site, 703 Vine Street, West Newton, PA: On or about August 11, 2025 during roofing work, the employer did not ensure an employee used appropriate fall protection systems/devices where fall hazards of up to approximately 20 feet existed.   b) Job site, 703 Vine Street, West Newton, PA: On or about August 12, 2025 during roofing work, the employer did not ensure an employee used appropriate fall protection systems/devices where fall hazards of up to approximately 17 feet existed.

29 CFR  1926.20(b)(2):  A program was not provided for frequent and regular inspections of the job sites, materials, and equipment to be made by competent person designated by the employer:  a) Job site, 703 Vine Street, West Newton, PA: On or about August 12, 2025 during roofing work, the employer did not ensure a system of hazard identification and correction was in use. Deficiencies noted during the inspection included, but were not limited to, failing to use fall protection systems and devices when working at heights over 6 feet, not using safety glasses while drilling into metal with power tools and failing to ensure that ladders used to get up and down to and from the roof were extended at least 3 feet above the landing.
