29 CFR 1926.20(b)(2): A program was not provided for frequent and regular inspections of the job sites, materials, and equipment to be made by competent person designated by the employer: a) Job site, 703 Vine Street, West Newton, PA: On or about August 12, 2025 during roofing work, the employer did not ensure a system of hazard identification and correction was in use. Deficiencies noted during the inspection included, but were not limited to, failing to use fall protection systems and devices when working at heights over 6 feet, not using safety glasses while drilling into metal with power tools and failing to ensure that ladders used to get up and down to and from the roof were extended at least 3 feet above the landing.
