Deficiencies noted during the inspection included, but were not limited to, failing to use fall protection systems and devices when working at heights over 6 feet, not using safety glasses while drilling into metal with power tools and failing to ensure that ladders used to get up and down to and from the roof were extended at least 3 feet above the landing.
