29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) On or about 08/12/2025, in the Spiral Kitchen, employees were exposed to amputation hazards where guarding and the enforcement of operating procedures were not adequate to protect employees from reaching into the AMFEC CO2 Mixer while it was running to remove product from the exit ports. An employee suffered a multi-finger amputation after reaching into an exit port as the mixer was operating. ABATEMENT DOCUMENTATION REQUIRED
