29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person: On or about August 8, 2025, employees working from a raised man basket were exposed to struck-by and crushing hazards while demolishing a concrete mixing hopper and the employer did not ensure a competent person inspected the worksite and determine the weight of the concrete hopper before or during its removal.
