29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials, and equipment to be made by a competent person: On or about August 13, 2025, employees were exposed to fall hazards while performing construction activities, including working from a concrete wall taking measurements because the employer did not ensure a competent person conducted frequent and regular inspections of the worksite and equipment to identify the workplace hazards.

29 CFR 1926.501(b)(1):Unprotected sides and edges. Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about August 13, 2025, employees were exposed to a fall hazard of approximately 8 feet while performing construction activities, including taking measurements along the top edge of a concrete wall because adequate fall protection was not being used.
