29 CFR  1926.501(b)(10):  "Roofing work on Low-slope roofs." Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width (see Appendix A to subpart M of this part), the use of a safety monitoring system alone [i.e. without the warning line system] is permitted.  On August 14, 2025, at a jobsite located at 409 South West Street, in Plainfield, Wisconsin;  (a)  On the west side of the building, back of the roof, employees were exposed to fall hazards of 13 feet 8 inches while pulling up a sheet of metal to the roof surface, on a low slope roof, without the use of fall protection.  (b)  On the south side of the building, the side canopy roof, employees were exposed to fall hazards of 14 feet while conducting roofing activities on a low slope roof without the use of fall protection.  (c)  On the east side of the building, the front canopy roof, employees were exposed to fall hazards of 13 feet 4 inches while conducting roofing activities on a low slope roof without the use of fall protection.

29 CFR  1926.502(h)(1): The employer shall designate a competent person to monitor the safety of other employees and the employer shall ensure that the safety monitor complies with the following requirements:  On August 14, 2025, at a jobsite located at 409 South West Street, in Plainfield, Wisconsin; during the installation of roofing material on an existing commercial building, the employer failed to ensure the safety monitor was on the same walking/working surface and within visual sighting distance of the employees being monitored [per 1926.502(h)(1)(iii)].  The employer failed to ensure the safety monitor was close enough to communicate orally with the employee [per 1926.502(h)(1)(iv)].  The employer failed to ensure the safety monitor had no other responsibilities which could take the monitor's attention from the monitoring function [per 1926.502(h)(1)(v)].  The safety monitor was observed performing roofing activities, measuring activities, on different levels than those being monitored, and not close enough to communicate orally.  As a result, employees working near the roof edge were not effectively monitored and were exposed to fall hazards between 13 feet 4 inches and 14 feet.

29 CFR  1926.1051(a):  A stairway or ladder shall be provided at all personnel points of access where there is a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist is provided.  On August 14, 2025, at a jobsite located at 409 South West Street, in Plainfield, Wisconsin; employees were exposed to trip and fall hazards of approximately 29 inches when accessing/egressing the south side of the building side canopy roof while performing roofing activities.

29 CFR 1926.503(b)(1):  The employer shall verify compliance with paragraph (a) of this section by preparing a written certification record. The written certification record shall contain the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer. If the employer relies on training conducted by another employer or completed prior to the effective date of this section, the certification record shall indicate the date the employer determined the prior training was adequate rather than the date of actual training.  On August 14, 2025, at a jobsite located at 409 South West Street, in Plainfield, Wisconsin; the employer did not certify that employees exposed to fall hazards between 13 feet 4 inches and 14 feet, while conducting commercial roofing activities at various locations on the roof, had received training in the recognition of fall hazards and methods to mitigate those hazards prior to exposure.
