29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) Bead Wire Department; Finished Goods Packing Station: On August 1, 2025, and at times prior, the employer exposed employees to struck-by and caught-in hazards in that the employer did not implement a method of machine guarding to protect operators from entering the path of the automated Demag DG-TR-DM, 1.25-ton overhead crane as it is picking up and moving the load.

29 CFR 1910.305(g)(2)(iii):Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws: a) Bead Wire Department; Finished Goods Packing Station: On August 13, 2025, and at times prior, the employer exposed employees to electrical hazards, in that the employer did not ensure that the Trumpf TruTool wire cutter was connected to fittings such that strain relief was provided as employees used the tool to cut quality control samples of steel wire.
