29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provided for an alternative fall protection measure: a) Job site, 1708 Termon Avenue, Pittsburgh, PA: On or about August 15, 2025 during soffit installation activities, the employer did not ensure an employee on the roof used appropriate fall protection systems/devices where fall hazards of up to approximately 20 feet existed. FZL Remodeling was previously cited for a violation of this occupational safety and health standard, or its equivalent standard, 29 CFR 1926.501(b)(13) which was contained in OSHA inspection number 1747298, citation number 1, item number 1, and was affirmed as a final order on August 2, 2024, with respect to a workplace located at 7541 Hutchison Street, Pittsburgh, PA 15218.

29 CFR 1926.20(b)(2): A program was not provided for frequent and regular inspections of the job sites, materials, and equipment to be made by a competent person designated by the employer: a) Job site, 1708 Termon Avenue, Pittsburgh, PA: On or about August 15, 2025 during soffit installation activities, the employer did not ensure a system of hazard recognition and abatement was implemented at the site. Deficiencies noted during the inspection included, but were not limited to, not using fall protection, ladder labels being marred/faded/missing and an employee not using head protection when needed.
