29 CFR 1926.304(i)(1): Each circular hand-fed rip saw was not guarded by a hood which completely enclosed that portion of the saw blade above the table and that portion of the saw above the material being cut: Site - On or about June 23, 2025, two DeWalt table saws used to cut wood and trim were not guarded by a hood. Employees were exposed to amputation hazards.

29 CFR 1926.451(b)(5)(ii): The end of a platform greater than 10 feet in length extended over its support more than 18 inches and the platform was not designed and installed so that the cantilevered portion of the platform is able to support employees without tipping, or has guardrails which block employee access to the cantilevered end: Site - On or about June 23, 2025, employees working on scaffold platforms were exposed to falling up to approximately 20 feet to the ground below. The scaffold platforms extended approximately 24 inches from the support.
