29 CFR  1910.22(a)(1):All places of employment, passageways, storerooms, service rooms, and walking-working surfaces were not kept in a clean, orderly, and sanitary condition.  On or about August 19, 2025, at a jobsite  located at 900 North 69th Street in Kansas City,  Kansas, employees were exposed to trip hazards in  that the employer did not ensure the the working/walking surface was kept in a clean, orderly and sanitary condition. Electrical cords were left on the floor in paths of travel exposing employees to trip hazards.

29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:  On or about August 19, 2025, at a jobsite located at 900 North 69th Street in Kansas City, Kansas, employees performing painting and brushing activities utilizing 3M half mask respirators were exposed to respiratory hazards in that the employer did not provide appendix D information to employees voluntarily using respirators.

29 CFR 1910.134(h)(2)(i): Respirators were not stored to protect them from damage, contamination, dust, sunlight, extreme temperatures, excessive moisture, and damaging chemicals or were not packed or stored to prevent deformation of the facepiece and exhalation valves.  On or about August 19, 2025, at a jobsite located at 900 North 69th Street in Kansas City, Kansas, employees  were exposed to respiratory hazards in that the employer did not ensure that respirators used by employees were stored properly to protect from dust and /or chemical contamination. Respirators were observed placed on a moveable cart adjacent to chemicals including but not limited to Maskote Zinc Stop-off coating and Zenalube Moly lubricant which are a serious eye  and respiratory irritants.

29 CFR  1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:   On or about August 19, 2025, at a jobsite located at 900 North 69th Street in Kansas City, Kansas, employees were exposed to chemical hazards. Employees engaged in painting and sealing activities were exposed to inhalation hazards in that the employer had not provided employees with information and training on how to recognize, avoid and/or eliminate the hazards associated with chemicals including but not limited to Maskote Zinc Stop-Off coating, a category 1 flammable liquid and serious eye irritant, and Zenalube Moly lubricant, a serious eye and respiratory irritant.
