29 CFR 1910.212(a)(3)(ii):Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): On or about August 14, 2025, the point of operation on the foot pedal actuated Cincinnati Hydraulic Press Brake, Model #90CBII * 6ft, Serial Number 51016, was not properly guarded. The installed light curtain Black Max Model: LL-MAX-18-AS did not prevent the actuation of the press brake while operators were hand feeding parts 2 inches long by 1 inch wide and 1/4 inch thick in the danger zone of the press brake. Supplemental mechanical barrier guarding was not installed to prevent access over, under, or around the light curtain. The light curtain mode selector key was not secured and allowed modifications to the blanking and muting functions of the light curtain by unauthorized employees.
