Sprague Rd. Cleveland, OH 44134, employees were exposed to a fall hazard of approximately 11 feet while performing roofing activities on a residential home. Employees were utilizing a multi-position ladder configured as a twin-step ladder to access the roof, and the employer did not ensure the ladder was extended at least 3 feet above the upper landing surface.
