29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) On or about August 13, 2025, during a residential construction roofing project located at 1850 W. Sprague Rd. Cleveland, OH 44134, employees were exposed to a fall hazard of approximately 11 feet while performing roofing activities on a residential home. Employees were utilizing a multi-position ladder configured as a twin-step ladder to access the roof, and the employer did not ensure the ladder was extended at least 3 feet above the upper landing surface.
