You must submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the form. a) Three Guys Garage LLC dba The Roadster Shop, located at 28775 N Rt. 83, in Mundelein, IL - On or about June 08, 2025, the employer did not electronically submit information from calendar year 2024's OSHA Form 300A or equivalent by the specified date of March 2, 2025.
