29 CFR  1910.178(p)(1):Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been  taken out of service until restored to safe operating condition   On or about August 20, 2025, at the warehouse, employees engaged in material handling activities using multiple brands of forklifts were exposed to struck-by hazards in that forklifts were found to be in need of repair in the following instances:  a) Red Toyota model 50-8FGU25, serial # 80254, had cracked tires and no seat belt b) White Nissan, Model MAP4F2A25LV  cracked tires c) Red Toyota Model 8FGU25 Serial #79110 cracked tires and defective forks, ropes were used to hold the forks in place d) Yellow Komatsu model FG25t-16, serial # A414811, cracked tires
