29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): a.) On or about August 12, 2025, employees were exposed to machine hazards associated with moving parts when disassembling and reassembling the Ampco Model ZP1 Pump. The employer failed to ensure that energy control application steps were followed as the machine was not shut down or turned off to perform troubleshooting [per the 1910.147(d)(2) requirements]. As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from servicing hazards.

29 CFR 1910.147(f)(3)(i): A procedure was not utilized to afford the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device when servicing and/or maintenance was performed by a crew, craft, department, or other group. a) Production - On or around August 12, 2025, two employees performed service and maintenance on the Savage Bros. Co. Icing Melter Machine, Model 0972-35-EC480V3PH, Serial No. 09-5569 and Ampco Model ZP1 Pump, when an employee sustained fingertip amputations on the left index and middle fingers. A group lock out procedure was not utilized which afforded the employees a level of protection equivalent to that provided by the implementation of a personal lockout or tagout device.

Co. Icing Melter Machine, Model 0972-35-EC480V3PH, Serial No. 09-5569, Cooling Towers, and other operating equipment. The employer did not ensure periodic inspections of the energy control procedures were performed at least annually as required by the standard.
