29 CFR  1903.19(c)(1):  Within 10 calendar days after the abatement date, the employer must certify to OSHA (the Agency) that each cited violation has been abated, except as provided in paragraph (c)(2) of this section.  On or about January, 27, 2026, with respect to a worksite located at  803 Co Rd U in Wrightstown, Wisconsin, the employer failed to certify to OSHA the fall hazards related to four citations, 1-1a, 1-1b, 1-1c, 1-1d, issued on December 18, 2025, had been abated.

29 CFR  1926.451(a)(6):  Scaffolds shall be designed by a qualified person and shall be constructed and loaded in accordance with that design. Non-mandatory Appendix A to this subpart contains examples of criteria that will enable an employer to comply with paragraph (a) of this section.  On or about August 20, 2025, at 803 Co Rd U in Wrightstown, Wisconsin, the employer did not ensure that scaffold equipment was constructed as designed for an employee installing soffit materials 13 feet above surface level without the platform end at least 6 inches over the support, fall protection equipment installed, support ladder positioned on stable surface, and support ladders secured as a scaffold system.

29 CFR  1926.451(b)(4):  Each end of a platform, unless cleated or otherwise restrained by hooks or equivalent means, shall extend over the centerline of its support at least 6 inches (15 cm).  On or about August 20, 2025, at 803 Co Rd U in Wrightstown, Wisconsin, an employee was exposed to potential fall hazard while installing soffit materials working from a scaffolding stage without the northern end of the platform at least 6 inches over the ladder support rung.

29 CFR  1926.451(g)(1):  Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers.  On or about August 20, 2025, at 803 Co Rd U in Wrightstown, Wisconsin, an employee was exposed to a fall hazard while working from a scaffold platform at 13 feet above surface level installing soffit materials without using required fall protection equipment.

29 CFR  1926.454(b)(2):  The employer shall have each employee who is involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold trained by a competent person to recognize any hazards associated with the work in question. The training shall include the correct procedures for erecting, disassembling, moving, operating, repairing, inspecting, and maintaining the type of scaffold in question.  On or about August 20, 2025, at 803 Co Rd U in Wrightstown, Wisconsin, the employer did not provide required scaffold safety training for an employee who was exposed to a fall hazard greater than 10 feet while installing soffit materials from a scaffold platform.

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.  On or about August 20, 2025, at a worksite located at 803 Co Rd U in Wrightstown, Wisconsin, an employee was exposed to a fall hazard of 11 feet while installing soffit materials from a second-floor patio on a residential construction site without the use of conventional fall protection.
