OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the employees were exposed to struck-by and crushing hazards from collapsing storage racks due to missing bolts to secure the racks:  (a) Establishment, Warehouse area - On or about August 20, 2025, employees were exposed to struck-by and crush-by hazards while performing work activities such as, but not limited to operating a powered industrial truck (PIT) to move pallets of materials,  such as printers and copiers, on and off of a steel storage rack systems.  The employer did not ensure that all of the base plates of the rack columns (posts) were totally anchored to the warehouse floor with anchor bolts, and that the rack columns were free from damage.
