29 CFR  1926.100(a):Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  a.) At the worksite, located at 2307 E Busch Blvd Tampa, FL 33612; On or about 8/19/25, the employer exposed employees to struck by hazards in that head protection was not provided for employees who were at ground level as roofers who worked above were throwing roof shingles and debris to the ground.

At the worksite, located at 2307 E Busch Blvd Tampa, FL 33612; On or about 8/19/25, the employer exposed employees to fall hazards of approximately 16ft in that fall protection was not provided for employees conducting roofing operations on a commercial building.

At the worksite, located at 2307 E Busch Blvd Tampa, FL 33612; On or about 8/19/25, the employer exposed employees to fall hazards of approximately 16ft in that employees had not received any fall protection or fall hazard recognition training prior to conducting roofing operations on a commercial building.

At the worksite, located at 2307 E Busch Blvd Tampa, FL 33612; On or about 8/19/25, the employer exposed employees to fall hazards of approximately 16ft in that an extension ladder did not extend 3 ft past the landing surface for employees conducting roofing operations on a commercial building.
