In the Alternative: 29 CFR 1926.21(b)(2) The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury. Location - 451 First Street Boulevard, Lowell, MA: On or about 6/2/25, the employer failed to conduct an adequate personal protective equipment (PPE) assessment and train employees in order to minimize injury while manually rotating parts on a horizontal centrifuge.
