29 CFR 1926.701(b):All protruding reinforcing steel, onto and/or into which employees could fall, was not guarded to eliminate the hazard of impalement: a) 650 N Pike St, Grafton, WV: On or about August 19, 2025 and at times prior, employees were exposed to impalement hazards while working around vertically protruding reinforcing steel that was not equipped with protective caps or covers during underground conduit installation activities. No abatement certification or documentation is required.
