29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye: On or about July 25, 2025, an employee received injuries resulting in an in-patient hospitalization, while removing a grain spout on a commercial construction site in Elliott, Illinois. The employer did not report the in-patient hospitalization to the Occupational Safety and Health Administration (OSHA) within twenty-four (24) hours.
