29 CFR  1910.305(g)(1)(iii):Flexible cords and cables were used for purposes prohibited by subparagraphs (A) through (E) of this paragraph:  a)  200 West 7th Ave, Collegeville, ECM and Fab area rear exit door - Employees are performing work tasks in a shop area that contains flexible cords used for permanent wiring.  Energized extension cord secured to building's structure, over a pedestrian walkway, powering a vending machine on the other side.  b)  200 West 7th Ave, Collegeville, ECM and Fab area rear exit door - Employees are performing work tasks in a shop area with flexible cords utilized for permanent wiring.  Energized extension cord is run from a hallway between a door and frame, into another room to power a scale.  c)  200 West 7th Ave, Collegeville, ECM and Fab area - Employees are performing work tasks in a shop area with flexible cords utilized for permanent wiring.  An energized extension cord is secured to overhead building members and powers a machine that dispenses personal protective equipment.

29 CFR  1910.333(c)(2): The employer did not ensure that qualified persons were capable of working safely on energized circuits and were familiar with the proper use of special precautionary techniques, personal protective equipment, insulating and shielding materials, and insulated tools:  a)  200 West 7th Ave Collegeville, Shop Area - Maintenance employees are regularly assigned to perform work tasks involving energized parts and circuitry ranging from 50 to 480 volts.  The employer has not provided arc flash training to the employees performing this work.

Employee training has lapsed beyond the one year period.

29 CFR  1910.303(g)(1)(i)(A):For electric equipment operating at 600 volts, nominal, or less to ground, and likely to require examination, adjustment, servicing, or maintenance while energized, the depth of the working space in the direction of access to live parts (as measured from the live parts where they were exposed or from the enclosure front or opening whwere they were enclosed) was less than indicated in Table S-1 of Subpart S of Part 1910:  a)  200 W. 7th Ave Collegeville -  Employees are performing work tasks in a shop area.  An electrical disconnect was obstructed so that it could not be readily accessed for operation or examination.
