29 CFR 1926.453(b)(2)(iv): Employee(s) working in an aerial lift were not standing firmly on the floor of the basket: On or about August 20, 2025, at 25070 Country Club Blvd, North Olmsted, OH 44070, the employer failed to ensure employees performing framing work installing a parapet wall was protected from a fall of approximately 50 feet in that an employee climbed through the guardrail system onto the roof to come down to the ground.

29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: On or about August 20, 2025, at 25070 Country Club Blvd, North Olmsted, OH 44070, the employer failed to ensure employees performing framing work installing a parapet wall was protected from a fall of approximately 50 feet in that no personal fall arrest system was utilized. OJ Construction Company Inc, was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.453(b)(2)(v) which was contained in OSHA inspection number 1624490, citation number 01, item number 001 and was affirmed as a final order on 12/12/2022, with respect to a workplace located at 1688 Worcester Street, Framingham MA, 01701.
