29 CFR  1910.178(l)(1)(ii): The employer did not ensure that each operator had successfully completed the training required by paragraph (l), except as permitted by paragraph (l)(5), prior to permitting an employee to operate a powered industrial truck   At a worksite located at 27003 E Oakview Dr, Lake Lotawana, MO, employees were exposed to caught between, struck by and crushed by hazards while operating power industrial trucks. The employer did not ensure that employees who were operating powered industrial trucks in the workplace completed training consisting of a combination of formal instruction, practical training, and evaluation of the operator's performance in the workplace.

29 CFR  1926.102(a)(1):29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles.  At a job site located at 27003 E Oakview Dr, Lake Lotawana, MO, employees were exposed to eye and face injuries from flying particle hazards. Employees conducting residential framing work were observed using pneumatic nail guns to fasten lumber together without eye and face protection.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  At a job site located at 27003 E Oakview Dr, Lake Lotawana, MO, employees were exposed to fall hazards while conducting residential framing activities. Employees were observed working at heights greater than 6 feet to the next lower level without having adequate fall protection while installing trusses and various other framing work.
