29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace; a) Warehouse floor; On or about , August 20, 2025, and time prior, the employer exposed employees to struck-by hazards in that the required training contents of a Powered Industrial Truck Program was not provided to employees who operate the powered industrial trucks (Nissan Electric Forklift; (Model #CSP01L18S) on the warehouse floor. b) Warehouse floor; On or about , August 20, 2025, and time prior, the employer exposed employees to struck-by hazards in that the required training contents of a Powered Industrial Pallet Jack was not provided to employees who operate the powered industrial pallet jack (WP 3000 Series Electric Pallet Jack; Model #WP3035-45) on the warehouse floor. c) Warehouse floor; On or about , August 20, 2025, and time prior, the employer exposed employees to struck-by hazards in that the required training contents of a Manual Powered Pallet Jack was not provided to employees who operate the manual pallet jacks on the warehouse floor.

29 CFR 1910.305(g)(2)(iii):Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: (a) Warehouse floor; On or about , August 20, 2025, and time prior, the employer exposed employees to electrical shop hazards in that the power supply cord for the electrical pallet jack (Crown - #D-2745SAMFRBDF48211) was identified as being under stress/strain from being strung from the equipment and into the wall outlet. (b) Warehouse floor; On or about , August 20, 2025, and time prior, the employer exposed employees to electrical shop hazards in that the power supply cord at dock door number 1 was identified as being under stress/strain from being strung from the equipment and into the wall outlet.
