29 CFR 1926.652(a)(1):Each employee in an excavation shall be protected from cave-ins by an adequate protective system designed in accordance with paragraph (b) or (c) of this section: On or about August 22, 2025, during the installation of foundation footing rebar for the construction of an apartment building at a worksite located at 22603 Center Ridge Road, Rocky River, Ohio, the employer failed to ensure an employee working inside a trench, with a depth of approximately 6 feet, was protected from cave-ins by an adequately designed protective system. The soil would have been classified as: Cohesive Soil, Type "B".
