The Enviro-Rack Area - On or about August 19, 2025, the 100 gallon gas pumping tank against the metal pole-building was not separated or relief designed to keep flammable relief vapor away from the building resulting in the spread of fire exposing employees to fire and structural collapse struck-by hazards. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

Enviro-rack area - On or about August 19, 2025, employees were attempting to drain the secondary containment reservoir of the enviro-rack with an unrated sump pump and general service extension cord, which resulted in the ignition of flammable liquids and vapor from the gasoline and vehicle fluids drained on the rack resulting in fire and burn injuries. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
