29 CFR 1910.28(b)(1)(i): Except as provided elsewhere in this section 29 CFR 1910.28, the employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that is 4 feet (1.2 m) or more above a lower level is protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):   a) On or about August 21, 2025, employees were exposed to fall hazards ranging from approximately 8 to 15 feet to the lower level, in that an adequate fall protection system such as guardrails, safety nets, or personal fall arrest systems was not provided while they were engaged in metal work activities on large metallic ventilation structures.  b) On or about August 21, 2025, employees were exposed to fall hazards ranging from approximately 8 to 15 feet to the lower level, in that an adequate fall protection system such as guardrails, safety nets, or personal fall arrest systems was not provided to employees engaged in metal work activities while working from a scissor lift, and using the top guardrail to step onto the top of the large metallic ventilation structures.
