Location: Job site;  On or about 08/22/2025, employees were allowed to utilize pneumatic nail guns without proper eye/face protection to protect against potential flying objects.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b).  Location: Job site;  On or about 08/22/2025, an employee was allowed to perform siding work from the porch roof on a residential home that was approximately 14-feet above the ground below without any means of fall protection.

29 CFR 1926.451(g)(1)(vii): An employee on a scaffold, not otherwise specified in paragraphs (g)(1)(i) through (g)(1)(vi) of this section, more than 10 feet (3.1 m) above lower level was not protected from falls by the use of personal fall arrest systems or guardrail systems meeting the requirements of paragraph (g)(4) of this section.  Location: Job site;  On or about 08/22/2025, an employee was exposed to a fall of greater than 15-feet above the ground while working from a bracket scaffolding system that was erected without a complete guardrail system or fall protection system to protect them.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access nor was the ladder secured at it's top to a ridged support and a grasping device, such as a grabrail, provided to assist the employee in mounting and dismounting the the ladder.   Location: Job site;  On or about 08/22/2025, employees utilized a portable extension ladders to gain access to the upper levels of the structure that were being sided and the ladders did not extend at least 3-feet above the area which they were being used to gain access to.
