On or about August 21, 2025, at the above addressed jobsite, an employee using a pneumatic nail gun was not wearing safety glasses, thereby exposing the employee to hazards associated with eye injuries.

On or about August 21, 2025, at the above addressed jobsite, an employee performing residential carpentry operations, twenty three (23'), feet six (6) inches  above the ground, was not protected from falls, thereby exposing employee to fall hazards.
