29 CFR  1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person (i.e., a person who is capable of identifying and correcting hazards):  a) CraftSource -3075 Terwood Rd., Willow Grove, PA Worksite - On or about August 20, 2025, the general contractor did not maintain frequent inspections by a competent person at the jobsite, in that subcontractor employees were exposed to visible concrete (Silica) dust from Enhanced Concrete Surfaces employees using handheld grinders to level a concrete floor (Table 1 Task).  CraftSource did not ensure that Enhanced Concrete Surfaces properly implement the required engineering controls, work practices, or respiratory protection as specified in Table 1.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.  Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photograph or video evidence of abatement or other written records.

29 CFR 1926.1153(c)(1): For each employee engaged in a task identified on Table 1, the employer did not fully and properly implement the engineering controls, work practices, and respiratory protection specified for the task on Table 1, unless the employer assesses and limits the exposure of the employee to respirable crystalline silica in accordance with paragraph (d) of this section.  a) CraftSource -3075 Terwood Rd., Willow Grove, PA Worksite - On or about August 20, 2025, Enhanced Concrete Surfaces employees were using handheld grinders to level a concrete floor (Table 1 Task).  CraftSource, the controlling general contractor did not ensure that Enhanced Concrete Surfaces employees properly implement the required engineering controls, work practices, or  respiratory protection as specified in Table 1, and did not require Enhanced Concrete Surfaces to assess exposure limits to silica in accordance with paragraph (d) of this section, exposing the employee to silica inhalation hazards.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.  Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photograph or video evidence of abatement or other written records.
