OSH ACT of 1970 Section (5)(a)(1):  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall hazards involving a damaged structural metal post supporting an exterior stairway and landing:  On or about August 20, 2025, employees were exposed to fall hazards greater than 12 feet when utilizing an exterior fixed stairway and landing located on the east side of the separator building which was supported by a damaged structural post that was corroded, bowing, and displaying signs of metal fatigue involving a horizontal crack in the middle of the post.  This condition exposed employees to fall hazards of approximately 12 feet onto the lower level.

OSH ACT of 1970 Section (5)(a)(1):  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to falls while working near unguarded edges of walking working surfaces:  (a) On or about August 20, 2025, the employer did not ensure employees working directly adjacent to the edge of a concrete manure collection pit were protected from falls. This condition exposed employees to fall hazards of approximately 15 feet onto a concrete surface below and/or into collected manure below.  (b) On or about  August 20, 2025, the employer did not ensure employees working directly adjacent to the western edge of the second story separator building were protected from falls. This condition exposed employees to fall hazards of approximately 12 feet onto a concrete surface below.  (c) On or about August 20, 2025, the employer did not ensure employees working inside the conexes, located above the manure management system pump room, were protected from falls to a lower level.  The guard rail system above the stairwell was missing a midrail.  This condition exposed employees to fall hazards of greater than four feet to the lower level.
