29 CFR  1904.39(a)(2):Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA.  a. Norris Construction LLC: At a worksite located at 301 North Main St., Cottage 5, Bethesda, OH 43719, On or about August 18, 2025, the employer failed to report the hospitalization and amputation of an employee's left thumb, sustained while using a Makita Model 5007FA circular saw, to OSHA within the required 24 hours.
