29 CFR 1904.39(a)(2):The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye. (a) The U.S. Postal Service failed to report the hospitalization and subsequent fatality of a postal carrier which occurred on August 21, 2025 who was assigned to the Lake Stevens Annex. The U.S. Postal Service was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1904.39(a)(2), which was contained in OSHA inspection number 1718664, citation number 1, item number 1 and was affirmed as a final order on 3/28/2024, with respect to a workplace located at 7001 S. CENTRAL AVENUE, LOS ANGELES, CA 90052. The U.S Postal Service was previously cited for a violation of this occupational safety health standard or its equivalent standard 29 CFR 1904.39(a)(2), which was contained in OSHA inspection number 1694817, citation number 1, item number 1, and was affirmed as a final order on January 16, 2024, with respect to a workplace located at 300 N. 10th St. Killeen, TX 76540.
