(a) On or about August 22, 2025, at the facility located at N115W19028 Edison Drive in Germantown, Wisconsin, an employee did not ensure that the forklift's parking brake was engaged prior to exiting the cab. This exposed employee(s )to struck-by and caught-in-between hazards.
