1926.501(b)(1)  Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest system:  (a) On or about August 20, 2025, the employer did not ensure employees working directly adjacent to the edge of a concrete manure collection pit were protected from falls.  This condition exposed employees to fall hazards of approximately 15 feet onto a concrete surface below and/or into collected manure below.  (b) On or about August 20, 2025, the employer did not ensure employees working inside the conexes, located above the manure management system pump room, were protected from falls to a lower level.  The guard rail system above the stairwell was missing a midrail.  This condition exposed employees to fall hazards of greater than four feet to the lower level.
