29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation and/or nip points: a. At the facility located at 3545 New Kings Road, Jacksonville, Florida: On or about August 18, 2025, and at times prior, the employer exposed employees to an amputation hazard, in that the Slitter Head's Locking Mechanism did not have a guard installed to protect from the nip points produced by the locking mechanism as employees locked/unlocked the Slitter Head machine into it's base.
