At the jobsite located at 8910 Grant Street, Overland Park, Kansas, employees using a portable extension ladder to access the roof to perform roofing activities were exposed to fall hazards of approximately 19 feet from the roof to the ground. The side rails of the portable extension ladder did not extend at least 3 feet above the landing surface of the roof.
