29 CFR 1910.22(a)(1): The employer did not ensure that all places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition.  (a)	On or about August 27, 2025, the employer did not maintain passageways, storerooms, and walking-working surfaces in a clean and orderly condition. Specifically, J-Room located in the front office area. Empty boxes, pallets, and products clutter the aisleways between the storage racks and the passageways, creating walking-working surfaces of less than 12 inches, exposing employees to tripping hazards while performing their order-building tasks.  (b)	On or about August 27, 2025, the employer did not maintain passageways, storerooms, and walking-working surfaces in a clean and orderly condition. Specifically, Aisles D, E, F, and G located in the main warehouse. Pallets of products are positioned in the aisleways between the storage racks, creating a walking-working surface of less than 24 inches, exposing employees to tripping hazards while performing their order-building tasks.

29 CFR 1910.22(c): The employer did not provide, and ensure each employee uses, a safe means of access and egress to and from walking-working surfaces.  (a)	On or about August 27, 2025, the employer did not provide or ensure a safe means of access and egress for employees building orders in J-Room. Specifically, the aisleways between the storage racks and passageways were cluttered with empty boxes, pallets, and products, which created walking-working surfaces measuring less than 12 inches. As a consequence, access and egress were obstructed, exposing employees to tripping hazards while performing their order-building tasks.  (b)	On or about August 27, 2025, the employer did not provide or ensure a safe means of access and egress for employees building orders in the main warehouse. On or about August 27, 2025, the employer did not ensure that step ladders were used for their intended purposes. Specifically, stepladders were used as single or straight ladders in the closed position, without the metal spreader in the open position, to access and retrieve products from storage racks that ranged from 8 to 9 feet in height, exposing employees to fall hazards.

29 CFR 1910.30(b)(1): The employer did not train each employee on or before May 17, 2017 in the proper care, inspection, storage, and use of equipment covered by this subpart before an employee uses the equipment.  On or about August 27, 2025, the employer did not provide the required training to each employee regarding the proper care, inspection, storage, and use of ladders before allowing them to utilize this equipment. Specifically, employees utilized several ladders including the Werner 6006 Fiberglass 6 ft step ladder, the Werner 6208 Fiberglass 8 ft step ladder, the Uline 7-step mobile ladder stand, and the Uline 8-step mobile ladder stand to access products positioned on storage racks with heights ranging from 8 to 9 feet. Employees utilized step ladders as single or straight ladders in the closed position, without the metal spreader opened, when access storage shelves.

29 CFR  1910.178(l)(1)(i):The employer did not ensure that each powered industrial truck operator is competent to operate a powered industrial truck safely, as demonstrated by the successful completion of the training and evaluation specified in this paragraph (l):  On or about August 27, 2025, the employer did not ensure that employees operating the Toyota 8FBE15U Electric Powered Industrial Truck (Forklift) were adequately trained and evaluated for their competency in safely operating Powered Industrial Trucks prior to their operation.  As a result, employees were exposed to stuck-by hazards, including increased risks of accidents from improper operating techniques, potential collisions, and the danger of tipping over or losing control of the powered industrial truck.
