29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly. LOCATION: 549 New Haven Avenue, Milford, CT 06460 On or about August 27, 2025, and times prior to, the employer modified the use of a Lull Corporation forklift, model 844B affecting capacity and safe operation without the manufacturer's prior written approval by use of a wooden platform to the forks of the forklift. Capacity, operation, and maintenance instruction plates, tags, or decals were not changed accordingly.
