29 CFR 1910.29(b)(2): 29 CFR 1910.29(b)(2): The employer did not ensure that guardrail systems met the requirement that midrails, screens, mesh, intermediate vertical members, solid panels, or equivalent intermediate members that are installed between the walking-working surface and the top edge of the guardrail system as follows when there is not a wall or parapet that is at least 21 inches (53 cm) high. On or about August 25th, 2025, employees engaged in warehouse operations located at 14685 Grover Street, Omaha, Nebraska 68144, were exposed to fall hazards in that the guardrail system located at the top of the "G" room stairwell only contained a top rail and did not contain a midrail that would prevent an employee from falling down to the lower level.
