29 CFR  1910.212(a)(1):Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices, electronic safety devices, etc.  On or about August 18, 2025, an employee was operating a FlexArm Tapping Machine to tap pre-drilled holes in an 11-inch by 2-inch by 3/8-inch metal strip. While repositioning their hand, the employee's glove was caught around the rotating 1/4-20 tap bit resulting in a serious injury to the left pinky finger. The employer did not ensure that the rotating tap and/or chuck was guarded to protect employees from caught-in hazards.
